Here is what you should bring for your first visit.
To start, we need your teeth! We will also need:
Photo ID and insurance cards
If you have any recent dental X-Rays within 6 months
All information detailing the history of your medical and dental health
Health history forms can be filled out online prior to appointment or at your consultation appointment (Download new patient forms from here.)
Current medications you are taking
A plan to pay for co-pays and any out of pocket costs (Go here for an easy financial cost estimation tool.)
Your first visit will include X-rays, Photographs and 3D- Scan along with a full comprehensive clinical examination by our orthodontist.
